New Patient Information

For inquiries about becoming a new patient, please call our main office at (863) 508-1101 and follow the recorded instructions to reach our referral line. New Patient referrals can be faxed to (863) 299-6158.

We are Accepting New Patient by Referral Only:
Prior to being scheduled for a new patient appointment, we must have a referral on file from your Primary Care Physician. Upon receiving the referral, a staff member from our office will call you to schedule an appointment at one of our convenient locations.
Please have your Primary Care Physician fax a referral to our office at (863) 299-6158.

Before Your Appointment:
Please complete the New Patient Registration Forms and bring it with you to your appointment. If you need us to mail you the forms, please contact our office. Plan to arrive to your scheduled appointment 30 minutes early to complete the registration process.

What To Bring:

  • Insurance card(s) and a photo ID
  • Completed New Patient Registration Forms
  • Previous medical records, in your possession
  • All prescription medications(s) and supplements, in their original containers

Established Patients

To schedule an appointment at any of our two convenient locations, please call our main office at (863) 508-1101 and follow the recorded instructions to reach our scheduling line.

What To Bring:
Always bring your insurance card(s) with you to each and every office visit, even if you have had no change in coverage. Please bring a current Medication List that includes the medication name, dosage, and how many times a day for our review.

Prescription Refills

To request a prescription refill, please call our main office at (863) 508-1101 and follow the recorded instructions to reach our nurse and prescription refill line.

Leave a detailed message including patient name and date of birth, medication and dosage, and pharmacy name and location. Please allow 48 to 72 hours for your prescription to be refilled.

Medical Records

To request medical records, please call our main office at (863) 508-1101 and follow the recorded instructions to reach our medical records line. Medical record requests can be faxed to (863) 299-6158.

Please complete our Medical Records Authorization Form to obtain your medical records from our office or you may use our form to have your medical records sent to our office.

No Show Policy

To reschedule or cancel an appointment, please call our main office at (863) 508-1101 and follow the recorded instructions to reach our scheduling line.

At KSC Cardiology, we understand that it may be necessary to reschedule an appointment due to an unexpected circumstance. If you must cancel your appointment, we respectfully request 24-hour notice.

Please call our main office at (863) 508-1101 as soon as you know you will not be able to make your appointment. Please understand that appointment times are limited. Your advanced notice will allow another patient access to that appointment time. Please read our No Show Policy prior to your visit.
